Хотелось бы узнать: Возможно ли сохранение жизни плода при оказании СЛР беременной женщины на поздних сроках беременности при сохранении жизни женщины? (Если да, то как?.... Прошу так же рассказать особенности СЛР у беременных на поздних сроках? (Например ..лекарственные препараты, которые представлялось вводить (учитывая влияние на плод), положение беременной, что в это время делать с плодом)...

Ну почему ж... Как думаете без кесарево обойтись можно?....

INTRODUCTION: The 2000-2002 triennial UK Report on Confidential Enquiries into Maternal Deaths concluded that over 50% of maternal deaths involved substandard care and that many could have been prevented. Catastrophic events leading to cardio-respiratory arrest may necessitate the resuscitation of pregnant women in various hospital locations. This study was designed to evaluate knowledge about resuscitation of parturients among anesthesiologists, obstetricians and emergency physicians. METHODS: A 12-question survey was distributed ######ly to residents and faculty in the anesthesia (ANES), obstetrics (OB), and emergency medicine (EM) departments at Stanford University Medical Center/Lucile Packard Children's Hospital, Stanford, California. Questions were designed to elicit knowledge deficiencies in four critical areas: need for left uterine displacement (LUD), advanced cardiac life support algorithms (ACLS), physiologic changes of pregnancy (PHYS), and the recommendation to perform cesarean delivery in parturients (>20 weeks gestation) after 4-5 min of unsuccessful resuscitation for cardiac arrest (5CD). RESULTS: In total, 74/75 physicians (43% ANES, 37% OB, and 20% EM) completed the test. ANES scored highest in overall test scores, and in knowledge of PHYS (P<0.05). Scores for LUD and 5CD were similar among groups, but 25-40% of these questions were answered incorrectly. In the ACLS category, the EM group scored highest (93%). CONCLUSION: We conclude that knowledge of important basic concepts, including the need for LUD and the potential benefit of early cesarean delivery during cardiac arrest, is inadequate among all three specialties. All three departments should provide ACLS physician training with emphasis on the special considerations for parturients.

А тут как бы плод не убить?....

Случись ситуация , про плод мало думать будите думаю.

Мать или "мать" спасать надо в первую очередь .

"Если, несмотря на смещение матки, стабилизации гемодинамики достичь не удается, абдоминальное родоразрешение должно быть выполнено немедленно. Не следует пытаться отложить кесарево сечение до "стабилизации" состояния матери так,как эта стабилизация не наступит без родоразрешения и устранения аорто-кавальной компрессии. Быстрое родоразрешение поможет реанимации и восстановлению матери. В дополнение к устранению аорто-кавальной компрессии, родоразрешение улучшит податливость грудной клетки, снижающуюся при беременности. Это дает положительный эффект как для закрытого массажа сердца, так и для проведения ИВЛ. Анализ результатов кесарева сечения, выполненного на фоне агонального состояния либо постмортально, показывает важность этой процедуры для новорожденного. Большинство детей, выживших в условиях столь экстренного кесаревого сечения, были извлечены в течении 5 мин после остановки сердца матери. При времени родоразрешения, превышающего 5 мин, шансы на выживание плода резко снижаются, значительно повышается риск тяжелых неврологических нарушений. В I и II триместрах беременности аорто-кавальная компрессия, создаваемая маткой, может быть минимальной, и экстренное абдоминальное родоразршение может не улучшить результатов реанимации"

Да в том то и вопрос: Обоих можно или нет?...(учитывая не литературные данные, а практику... может кто-нить смог, а?)....

За мать - посадят...А за плод - нет. В этом самый смысл...У матери - паспорт...Она гражданка, едрёна корень...А у плода пачпорт есть? Нэт.Плод - не человек...Юридическая тонкость, а вот как...

Коллеги, проведение СЛР у беременной на поздних сроках (или роженицы) имеет ряд особенностей, о которых писали выше. Во-первых, аортокавальная компрессия, пока она не будет устранена, не позволит на обеспечить даже те самые 30% МОК, которые получаются при грамотном проведении массажа сердца небеременному человеку. Во-вторых, как это уже было замечено, имеется высокое стояние диафрагмы, что также снижает эффективность ИВЛ, особенно если больная не интубирована. В-третьих, у беременных на поздних сроках, независимо от времени последнего приема пищи, имеется полный желудок, что резко увеличивает риск регургитации и аспирации, особенно если больная не интубирована. Сама же интубация у беременных часто бывает технически сложна, отмечается высокий процент трудных интубаций. Все лекарственные препараты, показанные при оживлении, вводятся без учета их пагубного действия на плод.

 

Вывод из всего этого один - беременная на поздних сроках в состоянии клинической смерти должна быть немедленно оперативно родоразрешена, так как только после этого можно будет говорить об эффективной реанимации и спасении жизни плода. СЛР нельзя выполнять в положении на спине. Либо подкладывают подушку под правое бедро, либо один из членов реанимационной бригады становится на колени, плотно опираясь стопами в пол, а пациентку поворачивают на левый бок, причем ее спина опирается на бедра стоящего на коленях; таким образом последний выполняет роль подпорки.

 

По поводу приоритета матери и новорожденного. Всегда приоритет отдается матери, за исключением случаев наличия у нее несовместимой с жизнью травмы. Никогда не идите в этом плане на поводу у некоторых акушеров.

Вы меня простите, но не очевидно ли:

1. При наличии СМПшных ситуаций кесарить тупой ложкой и набором для коникотомии-это турма. Если беременная нуждается в СЛР-заниматься СЛР. И не отвлекаться.

2. Если имеет быть НЕ СМПшная ситуация як то такой сферический конь, как развитие СЛР "на столе" у заинтубированной беременной и совершенно случайно пробегающая мимо бригада акушеров-гинекологов-то тогда да, экстренное кесарево можно обсуждать.

Resuscitation of the Pregnant Patient

 

Key Points:

 

During resuscitation there are two patients, mother & fetus

 

The best hope of fetal survival is maternal survival

 

Consider the physiologic changes due to pregnancy

 

Interventions to Prevent Arrest:

 

To treat the critically ill pregnant patient:

 

Place the patient in the left lateral position

 

Give 100% oxygen

 

Establish IV access and give a fluid bolus

 

Consider reversible causes of cardiac arrest and identify any preexisting medical conditions that may be complicating the resuscitation

 

Resuscitation of the Pregnant Woman in Cardiac Arrest:

 

Modifications of Basic Life Support

 

At gestational age of greater than 20 weeks, the pregnant uterus can press against the IVC & aorta, impeding venous return and cardiac output

 

Uterine obstruction of venous return can produce prearrest hypotension or shock and in the critically ill patient may precipitate arrest

 

It also limits the effectiveness of chest compressions

 

Modifications of Basic Life Support:

 

The gravid uterus may be shifted away from the IVC & aorta by placing in LUD or by pulling the gravid uterus to the side

 

This may be accomplished manually or by placement of a rolled blanket or other object under the right hip and lumbar area

 

Modifications of Basic Life Support:

Airway

 

Hormonal changes promote insufficiency of the gastroesophageal sphincter, increasing the risk of regurgitation.

 

Apply continuous cricoid pressure during positive pressure ventilation for any unconscious pregnant woman

 

Secure the airway early in resuscitation

 

Use an ETT 0.5 to 1 mm smaller in internal diameter than that used for a nonpregnant woman of similar size because the airway may be narrowed from edema

 

Breathing

 

Hypoxemia can develop rapidly because of decreased FRC & increased O2 demand, so be prepared to support oxygenation & ventilation

 

Ventilation volumes may need to be reduced because the mother’s diaphragm is elevated

 

Circulation

 

Perform chest compressions higher, slightly above the center of the sternum to adjust for the elevation of the diaphragm & abdominal contents

 

Vasopressor agents, including epinephrine & vasopressin, will decrease blood flow to the uterus, but since there are no alternatives, indicated drugs should be used in recommended doses

 

Defibrillation

 

Defibrillate using standard ACLS defibrillation doses

 

There is no evidence that shocks from a direct current defibrillator have adverse effects on the heart of the fetus

 

If fetal or uterine monitors are in place, remove them before delivering shocks

 

Differential Diagnoses

 

Same reversible causes of cardiac arrest that occur in nonpregnant women can occur during pregnancy

 

Providers should be familiar with pregnancy specific diseases & procedural complications

 

Use of abdominal US should be considered in detecting possible causes of the cardiac arrest, but this should not delay other treatments

 

Excess magnesium sulfate

 

Iatrogenic overdose is possible in women with eclampsia, particularly if the woman becomes oliguric

 

Administration of calcium gluconate (1 amp/1 g) is the treatment of choice

 

Empiric calcium administration may be lifesaving

 

Acute coronary syndromes

 

Pregnant women may experience ACS, typically in association with other medical conditions

 

Because fibrinolytics are relatively contraindicated in pregnancy, PCI is the reperfusion strategy of choice for STEMI

 

Pre-eclampsia/eclampsia

 

Pre-eclampsia/eclampsia develops after the 20th week of gestation & can produce severe HTN & ultimate diffuse organ system failure

 

If untreated it may result in maternal and fetal morbidity & mortality

 

Life-threatening PE & stroke

 

Successful use of fibrinolytics for a massive, life-threatening PE & ischemic stroke have been reported in pregnant women

 

Trauma and drug overdose

 

Pregnant women are not exempt from the accidents & mental illnesses

 

Domestic violence also increases during pregnancy; homicide & suicide are leading causes of mortality during pregnancy

 

Aortic dissection

 

Pregnant women are at increased risk for spontaneous aortic dissection

 

Alberta woman fine after giving birth during emergency heart surgery

Surgery was done Jan. 24, hours after the 35 week primip complained of SOB & Echo showed a thoracic aneurysm

 

Cardiac Sx opened chest and monitored mother’s heart while the Obstetrical team delivered the baby

 

After the obstetrics team delivered the child, Cardiac Sx completed the aorta graft

 

It was the first such procedure carried out in the region and only one of a handful done around the world

 

Emergency Cesarean Delivery for the Pregnant Woman in Cardiac Arrest:

 

CPR leader should consider the need for an ER cesarean delivery as soon as a pregnant woman develops cardiac arrest

 

The best survival rate for infants 24-25 weeks in gestation occurs when the delivery of the infant occurs no more than 5 minutes after the mother’s heart stops beating

 

This typically requires that the provider begin the delivery about 4 minutes after cardiac arrest

 

Delivery of the baby empties the uterus, relieving both the venous obstruction and the aortic compression

 

Delivery also allows access to the infant so that newborn resuscitation can begin

 

It is important to remember that you will lose both mother & infant if you cannot restore blood flow to the mother’s heart

 

Decision Making for Emergency Cesarean Delivery:

 

Consider gestational age

 

Although the gravid uterus reaches a size that will begin to compromise aortocaval blood flow at approximately 20 weeks of gestation, fetal viability begins at approximately 24 to 25 weeks

 

Portable US, may aid in determination of gestational age & positioning, but the use of US should not delay the decision to perform delivery

 

Gestational age less than 20 weeks

 

Need not be considered because this size gravid uterus is unlikely to significantly compromise maternal cardiac output

 

Gestational age approximately 20 to 23 weeks

 

Perform to enable successful resuscitation of the mother, not the survival of the delivered infant, which is unlikely at this gestational age

 

Gestational age greater than 24 weeks

 

Perform to save the life of both the mother & infant

 

The following can increase the infant’s survival:

 

Short interval between the mother’s arrest & the infant’s delivery

 

No sustained prearrest hypoxia in the mother

 

Minimal or no signs of fetal distress before the mother’s cardiac arrest

 

Aggressive & effective resuscitative efforts for the mother

 

Delivery to be performed in a medical center with a NICU

 

Consider the professional setting

 

Are appropriate equipment and supplies available?

 

Is emergency hysterotomy within the rescuer’s procedural range of experience & skills?

 

Are skilled neonatal support personnel available to care for the infant, especially if the infant is not full term?

 

Are obstetric personnel immediately available to support the mother after delivery?

 

Summary

 

Successful resuscitation of a pregnant woman & survival of the fetus require prompt & excellent CPR with some modifications in techniques

 

By the 20th week of gestation, the gravid uterus can compress the IVC & aorta, obstructing venous return & arterial blood flow

 

Rescuers can relieve this compression by positioning the woman on her side or by pulling the gravid uterus to the side

 

Defibrillation & medication doses used for resuscitation of the pregnant woman are the same as those used for other adults

 

Rescuers should consider the need for ER Caesarian Delivery as soon as the pregnant woman develops cardiac arrest

 

Rescuers should be prepared to proceed if the resuscitation is not successful within 4 minutes

Для начала нужно как минимум 2 человека: один занимается ребенком, а другой матерью. Без Кесарева не обойтись так как в литературе пишется что кесарево необходимо не только чтобы спасти ребенка, но и спасти мать. Если книническая смерть наступила, то хорошего качества НМС необходим до кесарева. Когда делаете НМС (если на полу) положите свою правую ногу больной по правое ребро. Таким образом плод как бы перекинется на левую сторону и етим вы снимите напряжение с правой венокавы (или как ето порусски), что провоцирует лучшему кровообращению при НМС.

У ребенка есть неплохоч шанс выжить если кесарево сделали в течении 5 мин после наступления клинической смерти и приситсвии хорошо обученного персонала. Выживит ли мать часто зависит от причины клинической смерти.
